Bravo Report

The work at EPAC is done by a two-person staff, a board of directors and lots and lots of volunteers. It’s very easy to miss something that one person did very well. The purpose of this form is to let you recognize something that someone did at EPAC that was well above and beyond what was needed or expected.

If you encounter someone at EPAC who has done an outstanding job of something, we’d like you to tell us about it. Some notes:

We’ll notify the person of your report. (Maybe we’ll also be able to publicize it further. That’s not decided yet).
If you agree, we’ll include a summary of your “Bravo Report” on our “Thank You” page on this web site (minus e-mail address(s) and phone number(s). (You can find the page at ‘Home > About Us > Thank Yous’
If you agree, we’ll include your name as the person who recognized the accomplishment.
